You can complete the referral form below yourself, or ask your GP, Support Coordinator, Nurse Navigator, family member, or another support person to complete it on your behalf.
If you’d prefer to speak with someone directly, call us on 0428 244 211 and we can guide you through the process over the phone.
Self-referrals are always welcome.
If you are privately funding your services or self-managed, a formal referral is not required.
Once we receive your referral, one of our friendly team members will be in touch within 1–2 business days. Depending on the service you require and appointment availability, we can often arrange your first appointment within a week.
